Name of Florist and will they be picked up or delivered to CCC? Date/Time of anticipated delivery? (Flowers must be delivered by noon to CCC on Saturday prior to the service on Sunday. We normally ask for a Saturday delivery around 10am, or otherwise between 9am and 12 noon.)

Order # Provided by Florist (Only required if florist will deliver, so that the Church Office can call to reconfirm the delivery a few days prior)
