Name of Florist who will deliver the flowers to CCC? Or will you deliver the flowers yourself? Date/Time of anticipated delivery? (Flowers must be delivered by noon to CCC on Saturday prior to the service on Sunday. We normally ask for a Saturday delivery around 10am, or otherwise between 9am and 12 noon.)

Order # Provided by Florist (Only required if florist will deliver, so that the Church Office can call to reconfirm the delivery a few days prior)

Donating flowers provides an opportunity for individuals to honor, celebrate, or memorialize a loved one.

Please fill out this form and click submit. Deadline to submit entry for Sunday's bulletin is Wednesday at 12pm on the week of flower donation.
